We have had overwhelming requests for the services our agency offers…. Because of the demand we are looking for additional staff in every area of our agency.

PFK Application

Family Home Provider (FHP)

Becoming an FHP is a commitment on the part of this staff member, this person would have someone, with varying degrees of disabilities that you are matched with to live in your home. We arrange a “meet and greet” with the consumer, case manager, family members, and close friends. This is done to ensure that you and your consumer are a “good fit”, however the final choice is consumer directed. The FHP can have up to three people living in their home with this program.

Direct Supports Provider

This staff person would work with a consumer to accomplish goals set by their case manager; these goals could include anything that your consumer would need to work on. This could include doing laundry, planning and preparing meals, and other tasks that would help these consumers incorporate inclusion within the community.

Respite Provider

These providers would give rest and relief to primary caregivers for folks with disabilities they care for. This is a valuable service for these caregivers and allows them to take a break from their normal routine while providing competent caregivers to care for the person living in their home.
