All services provided by Path Forward of Kentucky, as described below, are person centered. We strive to deliver high quality service to our participants and are always seeking compassionate, like-minded, experienced contractors as partners to provide those services. Due to the ongoing need for contracted providers, we are continuously accepting applications in both our Louisville and Frankfort offices.

If selected to provide contracted waiver services, additional training may be required for compliance with Medicaid Waiver Regulations.
